Hey! I would like to report a work injury. The accident happened yesterday to Judy Bowl, B-O-W-L, a 34 years old female worker. She is a machine operator, in the manufacturing department.
The accident happened in a corridor between the assembly lines and the toilets, at 2:08 pm yesterday.
I am the only witness of the accident, my name is Paul Barrack, B-A-R-R-A-C-K, I am her supervisor in the manufacturing department. I am the Manufacturing manager.
The accident happened when Judy was walking to the toilet and she slipped on the wet floor and fell down. She was only wearing safety shoes, and the cleaning lady did not put any wet floor sign.
She sprained her left ankle, and was taken to the hospital. No First Aid was provided on site.
She was taken to the hospital because she could not walk, her ankle was very painful. We called 911 and she was taken to the hospital to get an X-Ray.
