A little bit slow please.
If you have American accent, it will be great.
Thanks so much.
Five questions of the Fairness Algorithm:
1. Did the individuals intend to cause harm?
2. Did they come to work drunk or impaired?
3. Did they do something they knew was unsafe?
4. Could two or three peers have made the same mistake in similar circumstances?
5. Do these individuals have a history of involvement in similar events?
3 methods of structured communication:
1. SBAR: One example of structured communication is SBAR (Situation-Background-Assessment-Recommendation)
2. Briefing. For instance, a respiratory therapist could say to his team, “We’re going to be giving a breathing treatment to Jim Hernandez. Here’s the plan… Does anyone have any concerns about that?” Briefings ensure that everyone knows what’s supposed to happen – and thus make it a lot easier to spot things that shouldn’t be happening.
3. A debriefing is a different type of structured communication. It takes place after a procedure, and it can consist of three questions:
• What did we do well?
• What did we learn?
• What should we do differently next time?
The Features of a Culture of Safety
• Psychological safety. People know their concerns will be openly received and treated with respect.
• Active leadership. Leaders actively create an environment where all staff are comfortable expressing their concerns.
• Transparency. Patient safety problems aren’t swept under the rug. Team members have a high degree of confidence that the organization will learn from problems and use them to improve the system.
• Fairness. People know they will not be punished or blamed for system-based errors.
Thank you so much. Your accent sounds very pleasant to me!
